Ascend Reminder

We appreciate your patience moving forward as we continue our partnership with the Basecamp Health Ascend platform. Before our return to in-person learning, we’d like to provide you with a few friendly reminders in order to make this transition as smooth as possible:

Please ensure you have created your Ascend account. If you have not already done so, the temporary password used to create your account has expired. Please contact Basecamp Ascend Support (support@basecamp-health.com or (423) 443-4532) before September 4th in order to get this resolved.

Please add a shortcut to the Ascend website to the home screen of your mobile device. The shortcut  allows for quick access to the questionnaire. iPhone users, CLICK HERE for a video explaining how to add the shortcut to Ascend. Please read the questions carefully to ensure you answer each one correctly:
When answering the questions related to symptoms, only place a checkmark next to a symptom your child is experiencing if it is NOT related to another known cause. For example, a child could have sore muscles from a strenuous activity; in this case, the box should not be checked since the symptom  has another explanation.

Regarding the question about exposure and COVID testing, if you know your child has not had close contact with anyone who was contagious with COVID, please mark it as “Not Applicable.” Additionally, for the subsequent question about receiving a COVID test, if your child has not been tested, please mark as “Not Applicable.”

Several users have mistakenly entered the current day’s date in one or both of those questions, which causes an ineligibility result.

If you receive an invalid red ‘X’:
If you have completed the survey, the result shows as a red “X” or “ineligible”, and you believe this is in error, please contact Ascend@gsesdallas.org. Please provide the best means of contacting you, and a member of our team will follow up with you as soon as possible. to understand your situation and determine if exclusion from campus is required or if the error can be cleared with the student reporting to campus.
