The Clinic must collect all the necessary 2022-2023 health and medical forms prior to the first day of school, including the following:

Consent to Treat, Authorization, and Release - This form was included with annual enrollment/re-enrollment and it can be completed online, if you have not already done so.

Medical Certificate Form - This form is due annually and requires a physician's signature. This form is only valid for 12 months from the date of the physician’s signature (i.e., If the pediatrician signed the form on July 1, 2022 then it will be valid until June 30, 2023). Please take this form with you to your child's well visit and return a scanned copy to the Clinic via email (clinic@gsesdallas.org) or upload it to the SNAP Health Portal directly. 

Additional forms will be requested soon, along with applicable medications (Medical Permission forms, Asthma and Allergy Action Plans). Medication drop-offs will be available at the Clinic during “Meet the Teacher” days.
