IMPORTANT - Clinic Forms and Medication Drop Off

Dear Good Shepherd Parents,
We are excited to welcome your children back to campus next week and need your help to be ready for the new school year by providing the items listed below.
We still have many students needing to complete the “Consent to Treat, Authorization and Release” for the 2022-2023 school year. This form is required for all students in order to be on campus. A few weeks ago, an email entitled “Consent to Treat” was sent to all parents who have outstanding forms - if you did not receive this email, then your child does not have outstanding forms.
Depending on your child’s health needs, additional forms may be needed, including:

To reduce your trips to campus, we are scheduling medication drop-off times to coincide with Meet-the-Teacher events on Monday, August 22nd and Tuesday, August 23rd. We kindly request all parents who need to drop off medications do so on one of these days. 
If your child requires medication at school, please click this link to schedule a quick appointment to drop off medications. This includes daily medications, epi-pens, inhalers, and medications to be taken on an as-needed basis.
The medication drop-off appointments will occur in the CCL lobby, and we ask parents to schedule a time during their meet-the-teacher time slot. Students are not permitted to drop off their own medications or keep medications in their backpacks or lockers. The exception is for students with epi-pens and/or inhalers who have returned the appropriate, completed paperwork to the Clinic and have been approved to self-carry. Please bring medications in their original, unopened containers with the patient’s name and instructions. We cannot accept expired medication.
