On-Campus Voluntary Vaccination Opportunity December 9th


Once again, Good Shepherd will be partnering with EliteMed Health to host a voluntary vaccine clinic for our community, including students 5 to 11 years old. Vaccines are offered onsite as a convenience to members of the community who voluntarily choose to be vaccinated and are not required for any student. Please read below for important information and instructions if you would like for a member of your family to receive the COVID-19 vaccine on campus.
 
The date for this clinic was specifically set to allow children who participated in our  initial vaccine clinic on November 18th to follow up with their second shot of the pediatric Pfizer vaccination. We would also like to encourage any adult, including parents and faculty/staff, who are in need of a booster or any other COVID-19 vaccination to participate.

Registration: Please click here to complete the EliteMed Health Registration for your vaccine appointment. You must sign up ahead of time in order to receive the vaccine. 
 
Date: Thursday, December 9th
 
Time: 3:00-5:00 p.m.* 
*Disclaimer: Due to the high volume of participants at this event, some patients may be seen out of appointment time order. We will get everyone seen as quickly as possible. Please expect to be on site for a minimum of 30 minutes*
 
Location: CCL** 
**Parents picking up children after school should park, walk to pick up child(ren) at carpool and then proceed to the CCL together to attend the clinic.
 
Eligibility: Children ages 5 to 11 years old for Pediatric Pfizer vaccination, including the second shot for those that attended the November 18th on-site clinic. 

Additionally, children ages 12+ and all adults are welcome to attend and receive a full dose of the adult Pfizer, Moderna, or Johnson & Johnson vaccine or booster. Please note you may choose to receive your first dose of the vaccine at this clinic, however, your second dose (if necessary) should be obtained at another vaccine provider, such as EliteMed Health. At this time, Good Shepherd does not plan to host any future on-site vaccine clinics.

IMPORTANT NOTE: A parent or guardian must be present when the immunization is administered to your child. Depending on your child’s health history, plan to wait for 15 to 30 minutes after your child receives the vaccine.
